CEIH Heart Failure Survey System and Web Portal

Background
The Commission on Excellence and Innovation in Health (CEIH) launched a Heart Failure Subcommittee initiative to create a public-facing web portal and a platform for staff to engage with patients and stakeholders. The original site had become difficult to maintain due to a poorly documented and messy codebase.
Goal
The goal was to implement a survey system, similar to survey monkey, which could be used by clinicians and officials with CEIH to gather information from patients, doctors, and other stakeholders. It required a dynamic survey builder, which could fulfill a variety of question types and answers. Additionally, it required the integration of an authentication and authorization system, ensuring that only authenticated users with intended roles (patient, doctor, nurse, executive etc.) were targeted by surveys.
My Role
I was responsible for integrating a survey system into the portal. This allowed CEIH staff to design and distribute custom surveys, capturing feedback from clinicians, patients, and other key stakeholders. Given the state of the existing codebase, a full rebuild was necessary. I rebuilt the portal from scratch using technologies familiar to UniSA software engineering students, ensuring future contributors could work on the project efficiently.
Outcome
The new site was stable, maintainable, and fully documented. CEIH was pleased with the result, and the project is now positioned for long-term success.
